Supporting Your Move to Independence

At TC Housing Services, our Transitional Services are designed to assist individuals moving from an eligible setting to their own home or an integrated community support setting. We provide the essential support needed to make your move smooth and

What We Offer:

  • Moving Assistance
    We help with the physical aspects of moving, such as rental vehicle costs, packing, and unpacking, ensuring that your transition is as stress-free as possible.
  • Essential Furniture
    We cover up to $1,000 of essential furniture purchases, including items like bed frames, mattresses, sofas, dining tables, chairs, dressers, and lamps.
  • Household Supplies
    We provide up to $300 for necessary household items such as cleaning supplies, dishes, pots and pans, flatware, sheets, and towels.
  • Lease or Rental Deposits
    We assist with the payment of deposits for securing your new home.
  • Utility Set-Up Fees
    We cover utility deposits and setup fees to ensure you have access to necessary services like electricity, water, and internet.
  • Additional Covered Services
    • Pest and allergen treatment for your new home
    • Window coverings for privacy and comfort

Eligibility and Service Limitations

Transitional Services offer up to $3,000 per person, per transition, which can be accessed once every three years. The funding covers the essential items and services mentioned above, ensuring that you have the resources you need for a successful move.

Eligible Individuals

To qualify for Transitional Services, individuals must meet the following criteria:

  • Be 18 or older
  • Enrolled (or expected to enroll) in the Brain Injury (BI), Community Access for Disability Inclusion (CADI), Community Alternative Care (CAC), or Developmental Disabilities (DD) waiver
  • Moving from an eligible setting such as a foster care home, hospital, nursing facility, or other institutional settings
  • Unable to access these services from other funding sources
